YOU MUST NOT BOOK AN APPOINTMENT TO REGISTER UNTIL YOU HAVE RECEIVED A TELEPHONE CALL FROM THE MEDICAL EXAMINER TO CONFIRM THE MEDICAL CERTIFICATE HAS BEEN ISSUED.
Deaths should be registered within 5 days of the Registration Service receiving the Medical Cause of Death Certificate from the Medical Examiner.
If the death you need to register occurred within the City of Derby but you live in another local authority area, you may wish contact the registration office in the area where you live to enquire about making a declaration. This may mean the registration can be completed and certificates obtained without having to travel to Derby.
Medical Cause Certificates will be sent to us electronically by the Medical Examiner. They will inform you when this has been done. If you have been informed that the medical certificate has been sent electronically please answer 'Yes' to the question regarding medical certificates. Please contact the Medical Examiners Office at Royal Derby Hospital if you have any questions regarding this.
There may be delays to the registration if the death has or needs to be reported to the coroner as written clearance must be received by the registrar before the registration can take place.
For deaths within the Derby City Boundary the registrar will normally give you, free of charge, a form to allow the funeral to take place. Certificates will be available once the registration has been completed. Please bring a debit or credit card with you. The Registrar will also create a unique code to enable you to access and complete the Tell Us Once service either via their website or by telephone.
The Tell Us Once service allows a death to be reported to most Central and Local Government departments without having to contact each department individually.
